i'm guessing joint failure of some sort. i can't really see the joinery method with the clamp. lack of glue? poor machining?

Can you add some sort of a triangular brace to the inside face of the door? I'm thinking printed circuit board blank. Copper side out would look OK.

Plan B: Drill through the hardware to put in new dowels.

When working in the prototype (first build) shop of a furniture factory I once pointed out an obvious design flaw like this.
Of course the "designer" at the drafting table was way up the ladder from me, I was ignored.
They went on to build hundreds of filing cabinets with mortices 1/2" from the edge of particle board to hold them from racking out of square during minor collisions.
When they rapidly fell apart the whole line was discontinued over one guy who couldn't be wrong.
