Well that's not an inside-out turned ornament. It's just drilled. You can do that by removing 2 jaws from your chuck and mounting the blank so that the area where you want to drill the holes is pointing toward your tailstock. Then you can either use a drill chuck in your tailstock or you can simply turn the opening with a bowl or spindle gouge. Rotate it 90 degrees and do it again. You will have to put some scrap wood between the jaws and the piece to keep from marring it.
