Cost of arch for doorway opening

I have 4 different 5 feet wide openings (no doors) and I was wondering what is the additional cost to bump up and arch the top (only one is a supporting wall). Each opening is moving in location anyway so their will need to be a new header anyway so just looking at cost difference to get an arch in new openings. I know molding will be an additional cost as well. (I'm having a builder do the work in a major renovation and was wondering if we should add the curves to the new doorwells we are having shifted to the project)

Has anyone used easy-arch or elegant arches (elliptical unit nailed to header), corners framing block plywood and sheetrock so not structural corners?

Will these have wood jambs, or just be drywalled?
