Default Bed rail anchoring

I made a bunk bed and used threaded inserts. I counter sunk 3/8 bolts into 4x4 with threaded insert in the rail. For the most part works well. However one insert loosened and now will not seat. Want to find an insert with internal threads for 3/8 bolt which would expand inside the pocket hole on the rail. May well be there is no such piece. I have not yet found anything like that on the internet. Would appreciate any thoughts or direction.

They make what you want to be used in concrete. However, I think that the expansion principle may be a problem in wood. If you were to expand the hole then you may split the wood.

I have not done any research, but I think that you can find an insert that will thread into a bored hole and then you can screw the e/8 bolt into that.
